DATA SUBJECT APPLICATION FORM WITHIN THE SCOPE OF THE PERSONAL DATA PROTECTION LAW NO. 6698

Personal data owners, defined as “data subjects” under the Personal Data Protection Law No. 6698 (hereinafter referred to as “KVKK”), have been granted the following rights under Article 11 of the KVKK (hereinafter referred to as the “Applicant”):

  • To learn whether your personal data is being processed,
  • To request information if your personal data has been processed,
  • To learn the purpose of the processing of your personal data and whether they are used in accordance with their purpose,
  • To know the third parties to whom your personal data is transferred domestically or abroad,
  • To request the rectification of your personal data if it is processed incompletely or inaccurately, and to request the notification of this transaction to third parties to whom your personal data has been transferred,
  • To request the erasure or destruction of your personal data in the event that the reasons requiring its processing cease to exist—despite being processed in accordance with the KVKK and other relevant law and legislation provisions—and to request the notification of this transaction to third parties to whom your personal data has been transferred,
  • To object to the occurrence of a result against you exclusively through the analysis of the processed data solely by automated systems,
  • To demand the compensation of the damage in the event that you incur damage due to the unlawful processing of your personal data.

Pursuant to Article 13 (1) of the KVKK and the Communiqué on the Principles and Procedures for Request to Data Controller, applications to be made to our Company regarding these rights must be submitted in writing, or by sending an e-mail to iletisim@bridgeist.com using the electronic mail address previously notified to our Company by you and registered in our Company’s systems, or via other methods to be determined by the Personal Data Protection Board (hereinafter referred to as the “Board”) in the future.

Within this framework, applications to be made to our Company “in writing” can be submitted by printing out this form and via the following channels:

  • By the Applicant applying in person to our Company,
  • Through a notary public or by registered mail with return receipt requested,
  • Provided that the Applicant has an e-mail address registered in our Company’s systems, by sending an e-mail from this address to our Company’s address at iletisim@bridgeist.com,
  • By being signed by the Applicant with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070 and sent to the Company’s registered electronic mail address.

Below, specific information regarding the written application channels and how they should be delivered to us is provided:

Sending an E-mail from your e-mail address registered in our Company iletisim@bridgeist.com “Information Request Within the Scope of the Personal Data Protection Law” shall be written in the subject line of the e-mail.
Application with Secure Electronic Signature (Applying via Registered Electronic Mail (KEP) by signing with a secure electronic signature) xxxx@xxx.kep.tr “Information Request Within the Scope of the Personal Data Protection Law” shall be written in the subject line of the e-mail.

In addition, after other methods to be determined by the Board are announced, our Company will announce how applications will be received through those methods as well.

Pursuant to Article 13 (2) of the KVKK, your applications submitted to us will be responded to free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request, from the date your request reaches us. Our responses will be delivered to you in writing or electronically in accordance with the provisions of Article 13 of the KVKK.
